Dear Sir,

IC 29584Y Lt Col Vijay Kaushik, a life member of IESM, has contributed the sum of Rs. 5,000.00. Details are as follows:

1. Life membership fee - Paid life membership fee of Rs. 500.00 vide State Bank of India, Pune, demand draft No. 408573 dated 12 Feb 09, forwarded to Maj Gen Satbir Singh by First Flight Courier on 12 Feb 09.

2. Contribution - Rs. 5,000.00 (Rupees Five thousand Only) deposited in IESM account No. 06162000001330 at HDFC Bank, Kondhwa, Pune Branch, vide ICICI Bank, Kondhwa, Pune Branch, cheque No. 455902 dated 19 May 2009. The cheque was realized on 20 May 2009.

3. It is requested to forward details with copy to Col Kaushik and me.
